What is Traumatic Brain Injury?

Traumatic brain injury (TBI) occurs when an external force causes damage to the brain. This ranges from mild concussions to severe injuries resulting in permanent disability. In the United States, approximately 2.8 million TBIs occur each year, with an estimated 5.3 million Americans living with TBI-related disabilities.

Symptoms include persistent headaches, cognitive impairment, memory difficulties, sleep disruption, mood disturbances, and in severe cases, motor and sensory deficits. Many patients experience symptoms lasting months or years after the initial injury — a condition known as Post-Concussion Syndrome (PCS).

Quick Facts

How HBOT Helps TBI

Hyperbaric oxygen therapy works by delivering 100% oxygen at elevated atmospheric pressure (1.5–3.0 ATA). This dramatically increases the amount of oxygen dissolved in blood plasma — up to 15× normal — allowing oxygen to penetrate areas that red blood cells cannot reach due to swelling or vascular damage.

🔥
Reduces Neuroinflammation
HBOT downregulates inflammatory cytokines and reduces cerebral edema, the primary driver of secondary TBI injury.
🧠
Promotes Neuroplasticity
Elevated oxygen levels stimulate BDNF (brain-derived neurotrophic factor) and VEGF, promoting formation of new neural connections.
Improves Cerebral Metabolism
HBOT restores mitochondrial function in injured neurons, allowing cells in the 'ischemic penumbra' to resume normal activity.
🔬
Stem Cell Mobilization
Research shows HBOT significantly increases circulating stem cells, which may contribute to neural repair and regeneration.

Clinical Evidence

Selected peer-reviewed studies on HBOT for TBI. Evidence base continues to grow rapidly.

RCTPLOS ONE · 201356 patients
40 Sessions of HBOT Improve Cognitive Function in TBI Patients
Outcome: Significant improvement in memory, attention, and information processing
ProspectiveFrontiers in Neurology · 2018120 patients
Hyperbaric Oxygen Therapy for Chronic Traumatic Brain Injury
Outcome: 30% improvement in Post-Concussion Symptom Scale scores
Clinical TrialAging · 202028 patients
HBOT Induces Neuroplasticity and Improves TBI Outcomes
Outcome: Increased neuronal density on SPECT imaging; PTSD symptom reduction
ComparativeBrain Stimulation · 202291 patients
Repetitive Transcranial Magnetic Stimulation vs HBOT in Mild TBI
Outcome: Both effective; HBOT superior for headache and concentration
Note on evidence quality: While results are promising, TBI HBOT research faces challenges including heterogeneous patient populations, varying protocols, and placebo effects from hyperbaric pressurization. Consult with a hyperbaric physician who can evaluate your specific injury history and imaging.

Standard Treatment Protocol

1
Evaluation
1–2 weeks
Medical history review, neuropsychological testing, baseline SPECT or fMRI imaging. Physician determines appropriateness and designs personalized protocol.
2
Initial Phase (Sessions 1–10)
Weeks 1–2
Lower pressure (1.5 ATA) to assess tolerance. Most patients adapt quickly to pressure equalization. Some report early symptom improvement within this phase.
3
Core Treatment (Sessions 11–30)
Weeks 3–6
Full protocol pressure (1.5–2.0 ATA). 5 sessions per week, 60–90 minutes each. This is when the majority of therapeutic benefit occurs.
4
Integration Phase (Sessions 31–40)
Weeks 7–8
Continuation and consolidation of neurological improvements. Some patients extend beyond 40 sessions based on physician assessment and clinical response.
5
Follow-up & Reassessment
1–3 months post
Repeat neuropsychological testing and imaging to quantify improvements. Many physicians recommend a 'maintenance' protocol of 1–2 sessions per month.

💡 Tip: Many clinics offer significant package discounts (15–25%) and some offer payment plans. HSA and FSA funds can typically be used for HBOT. Ask about military/veteran discounts — many clinics serve veterans and offer reduced rates.
